Health Insurance Claim Submissions
How do I make a claim?
Prescription drugs may be purchased at the pharmacy of your choice. For easy, fast, and hassle-free service, you can choose to utilize one of the participating pharmacies where you are only required to pay the 20% co-insurance and we handle the rest!
For all other claims, or if you choose to utilize a non-participating pharmacy, you will required to pay for the full cost of the prescription or treatment and submit your original receipts to the Plan Administrator. A cheque will be mailed to you, usually within four weeks after submission.
** Claims are NOT processed through the MSU Main Office **

** This claim form is to be used for prescription reimbursement and accidental injuries (including accidental dental) ONLY. If you are making a dental claim, you must use the Claimsecure dental claim form located online at HERE
You may also obtain MSU Health Claim Forms at the MSU Main Office (MUSC 201).
Who is the Plan Administrator for the Health Plan?
Trothen & McConkey Insurance Brokers Ltd. is the Administrator for the MSU Health Insurance Plan and they adjudicate all health claim submissions.
