Coverage

COVERAGE INCLUDES:

Prescription Drugs     

 
When injury of sickness require prescription drugs, prescribed in writing by a physician or surgeon, the Plan will pay 80% of the actual cost of such prescription drugs or medicines, EXCLUDING CONTRACEPTIVES, up to a maximum of $1,400.00 during the policy year (September 1, 2011 to August 31, 2012).
 

AIDS Coverage

 
The plan will pay up to $5,000.00 with respect to drug treatments for HIV-positive or AIDS infected students.
 

Diabetes

 
Included within the prescription reimbursement limit of $1,400.00 is the cost of Insulin (bottled and/or cartridge), syringes and pen needles, reservoirs and infusion sets for the pump.  Glucometers, insulin pens, alcohol and swabs are NOT covered.  Financial assistance for purchasing glucometers may be available through the Canadian Diabetic Association.
 

Vision Care

 
One (1) eye examination will be reimbursed up to 50% of the cost, up to a maximum of $50.00.
 
If your glasses are accidentally broken, or if you rip your contacts (disposable contacts excluded), the MSU will reimburse 80% of the cost of repair of ONE (1) incident per year, up to a maximum of $100.00.
 

Life Insurance

 
The MSU will provide $5,000.00 life insurance benefit from sickness or accident.
 

Accidental Dismemberment

 
When an accidental injury results in any of the following losses, occurring within 365 days, the plan will pay the amount specified for such loss as per the schedule below.  Only one such loss (the greatest) shall be payable as the result of any one accident.

Quadraplegia, Paraplegia, or Hemiplegia

Ambulance Expense Benefit

 
When injury requires immediate medical attention, the Insurer will pay the customary charges for licensed ambulance services.  The maximum benefit for air and/or ground ambulance is $250.00 per occurence.
 

Repatriation Expense Benefit

 
If the Insured suffers an accidental loss of life outside Canada, the plan will pay the expense of homeward carriage of the body, subject to a maximum payment of $1,000.00.
 

Dread Disease Benefit Care

 
If Poliomyelitis, Scarlet Fever, Diphtheria, Spinal Meningitis, Encephalitis, Rabies, Teanus, Tularemia, Cancer, Typhoid, Hepatitis B, Non-A and Non-B Hepatitis which first manifests itself while the policy is in force with respect to the Insured and requires the Insured to be confined within a hospital or employ a licensed or graduate nurse within 156 weeks from the date that the disease is diagnosed, the plan will pay for the expense actually incurred, to a maximum of $5,000.00.
 

Out-Of-Province Medical

 
This policy offers limited coverage of $10,000.00 for accidental injuries only.  WE STRONGLY RECOMMEND that travel medical insurance be purchased if you travel outside of Ontario.
